Midlands Choice is a Midwest preferred-provider organization (PPO) with:
- • A network of more than 43,300 physicians and other licensed healthcare professionals.
- • 338 hospitals.
- • 2,405 ancillary healthcare providers.
The core network spans Nebraska and Iowa.

CoFinity
Available for groups size of 51+.
CoFinity is an independent Preferred Provider Organization (PPO), a network of physicians, hospitals, and other health care providers that have agreed to accept reduced service fees in order to provide affordable health care services. This network is available for those who live in Michigan, Northern Indiana and Northern Ohio.
Unlike most local PPOs that operate in a single state, our expanding network covers Michigan, Ohio, Indiana and parts of Kentucky, Wisconsin and Illinois.
